A short health history questionnaire, completed before your first appointment. It takes about five minutes and stays on your file for every visit after that.

This is a demonstration site, so the form itself is not live here. On a real booking you would be emailed a link after you book, or hand it in on paper at the desk five minutes early.
Contact and emergency contact details
Name, phone, email and one person to call if needed.
Current injuries or areas of pain
What is sore, since when, and anything that makes it better or worse.
Past surgeries
Especially anything in the last twelve months, or anywhere near where you want treatment.
Medications you take regularly
Blood thinners and anything affecting circulation matter most for pressure decisions.
Pregnancy status
Trimester if applicable, and any restrictions from your midwife or obstetrician.
Medical conditions
Diabetes, heart conditions, skin conditions and anything else relevant to massage.
Previous massage experience
What pressure you usually prefer and anything that has not worked for you before.
How you heard about us
Optional, and it does not change your treatment either way.
What to wear
Whatever you are comfortable in.
- Whatever you are comfortable undressing to. Most clients undress to their underwear for a full-body session.
- You stay fully draped throughout with a sheet and a towel. Only the area being worked is uncovered at any time.
- For a seated neck and shoulder session, you can stay fully clothed.
- Bring or wear loose clothing if you can, especially for a sports massage before or after training.
Draping, always
You are never fully undraped at any point. The sheet and towel move with the treatment, one area at a time, and you can ask for more coverage at any moment without it being awkward.
